Band 5 Substance Misuse Nurse – Clouds House

We are looking for a dedicated and compassionate nurse to join our team and make a real difference in the lives of individuals experiencing substance dependency. In this role, you will have the opportunity to work autonomously within our medical centre while also collaborating closely with a wider multidisciplinary team. You will be central to delivering high-quality, evidence-based care in line with national guidelines and Forward Trust policies.
This role offers you the chance to use your clinical expertise to provide holistic, recovery-focused care, supporting clients on their journey towards healthier lifestyles and improved wellbeing. You will play a key role in maintaining our treatment model and ensuring the ethos of the service is reflected in everything we do.
What You’ll Be Doing
- Conducting holistic assessments and providing evidence-based nursing interventions tailored to each client’s needs.
- Delivering brief interventions, crisis support, and care planning in line with recovery-focused principles, including the 12-Step model.
- Assessing and monitoring medical detoxifications and other prescribed medications.
- Supporting clients’ physical and psychological wellbeing, helping them navigate challenges and work towards positive lifestyle changes.
- Triaging clients for further medical assessment with an ANP, GP, or psychiatrist as needed.
- Working collaboratively with colleagues across the multidisciplinary team to provide integrated, high-quality care.
- Maintaining accurate, timely, and professional clinical records.
- Administering medication safely, including controlled drugs, and ensuring strict adherence to safe storage procedures.
- Following infection control practices and reporting any maintenance concerns.
- Treating clients with dignity, respect, and cultural sensitivity at all times.
- Participating in supervision, reflective practice, and professional development opportunities.
- Contributing proactively to team meetings, service development, and quality improvement initiatives.
What We Are Looking For
- Registered Nurse with valid NMC registration
- Sound clinical knowledge and nursing practice experience
- Experience in substance misuse or related field
- Strong assessment skills, including risk assessments
- Experience of key working and care planning
- Knowledge of recovery approaches (including 12-Step model)
- Robust knowledge of medication rounds, safe storage, and administration (including controlled drugs)
- Awareness of health issues relating to substance misuse
- Knowledge and understanding of infection control
- Ability to work effectively within a multidisciplinary team
- Strong IT and accurate record-keeping skills
- Good time management and organisational skills
- Evidence of ongoing professional development post-registration
- Ability to reflect on practice and engage in further training
- Commitment to treating service users with dignity, respect, and cultural sensitivity
- Strong communication skills with warmth and empathy
- Knowledge of maintenance reporting processes (desirable)
